By completing this form, you will be listed as the “Lead Instructor” for this course. As the Lead Instructor you have the following responsibilities

• Print and read full Submission Guidelines. Share this information with all additional instructors.
• Submit a complete “Workshop Instructor Proposal”, including full instructor list, course description, course outline/schedule and educational objectives.
• Distribute information regarding presentation to all listed instructors (i.e. acceptance, rejection, scheduling)
• Ensure you obtain permission for use of any copyrighted materials.

Please note: Workshop registration fees are waived for lead instructors and co-instructors only. No other expenses are covered by the Continuing Challenge (e.g. lodging, airfare, CEU credits, etc.)
